Advance care planning is for everyone—no matter where you find yourself on your health care journey. Whether you are a young adult or older adult, are healthy or have a chronic illness, now is the best time for you to start planning. What matters to you may change, so it is always a good idea to revisit advance care planning—throughout your life and health journey.
Let’s get our planning started together with the Fraser Health Authority Advance Care Team including Dr Wai Phan who will help us understand what “palliative care” is and how it differs from “hospice care”.

Fraser Health Authority Advance Care Team
Dr. Wai Phan is a Palliative Care Consultant for the Royal Columbia Hospital and community Palliative patients in New Westminster. His focus is on providing continuous and comprehensive primary medical care for individuals and families across all ages and stages of life.
Christine delos Santos is a Registered Nurse who began her career caring for adults before and after their surgical experience. She also worked in the primary care setting, collaborating with Family Doctors, where she gained notable experience in Advance Care Planning as well Serious Illness Conversations.
Christine’s priority as an Advance Care Planning Nurse Clinician is to empower both health care providers and the public to further enhance their confidence in making health care decisions that align with their values and wishes, for themselves as well as those who matter to them.
Vicky Kaler is a Registered Social Worker with over 15 years of experience within the social services field. In that time, she’s had the opportunity to work within a variety of settings including acute care, critical care, substance use services and long-term care.
In her work with individuals and their families, it has always been abundantly clear that helping patients and families identify what is important to them and having these conversations has been key in empowering individuals as they navigate the often complex medical world.
In her role as an Advance Care Planning Social Worker, Vicky hopes to engage with other health care providers and members of the public to normalize and highlight the importance of having conversations about your values, beliefs and what matters most to you.
