Bringing Advanced Care Planning to the unhoused community
In September 2021, New West Hospice Society joined with the city of New Westminster in a project entitled Voice for the Vulnerable.
The goal of the project was to meet with the homeless on the street, in coffee shops, in food lines, or panhandling or wherever they were and to have a conversation about their health, their family and what they wanted in terms of end of life care.
In addition, they discussed what they wanted done with their belongings should anything happen to them while they were living on the street.
All this information was recorded on what is called an Advance Care Plan and was filed with Fraser Health. The information was filed on the data base with the person’s Personal Health Care Number.
The project funded by the city ended in July of 2022.
Although the project ended, NWHS continued the work.
A volunteer continues to meet with the homeless on the street and at Connect Day where service agencies come together to offer support.
